Anime and Manga

An Anime and Manga of the same title may not have been written/drawn by the same person. Sometimes (if the manga comes first) an Anime might change a detail, a sequence, or an entire story because that's what they think will be better. Sometimes it's for a different target audience. Sometimes, the producers/directors/w.e just think it'll be better changed.
Sailor Moon manga is totally different from the Anime and more closely follows the Live Action Television show. Different target audience?
The Peacemaker Kurogane anime was a combination of Shinsengumi Imon Peacemaker and Peacemaker Kurogane.

A case of sticking too close to the Manga, I would say, would be Dragonball Z. There were long scenes of dramatic pause, which work in manga but not anime.

Sometimes people like to extend on a certain series so that the ending is more suited to their own liking. People even redo whole animes with the same characters as the original and just do things they like, with their own story. This creates lots of carbon copys of animes/manga with the same title, but not the same concept.
