Request Grim Reaper Avatar and Banner Request

What I want isn't that pacific but here is what I want:

On the Avitar I would like a closer zoom on Grims face and (if possible) My user name on it.

The Banner is much of the same except the size and what I want. Again I would like my name on the banner and if possible the phrase. "For an Instant, shock...and Fear" on it.

I will supply a attachment on the thread I hope some one will take this up.
